Client Participation Agreement
As a condition of receiving assistance through The Assistance Center of Collin County, all approved clients are required to sign up for and attend all classes directed by their case managerr. Failure to comply with this requirement may result in the denial or termination of assistance.
By signing below, I ackowledge and agree to the following terms:
I will attend all classes and programs assigned by my case manager as a part of my assistance plan.
I understand that attendance is mandatory, and failure to attend may result in the loss of assistance.
If I am unable to attend a scheduled class, I will notify my case manager in advance and discuss alternative options.
I ackowledge that my participation in these classes is essential for my continued elgibility for assistance.
